Emergency Asthma Kit/ Emergency Allergy Kit Consent Form

Schools are allowed to hold a spare emergency inhaler as part of a Schools Emergency Asthma Kit and a spare emergency Adrenaline pre-filled pen as part of a Schools Emergency Allergy Kit.

St George’s University Hospital has provided Sellincourt with a Schools’ Emergency Asthma Kit including a

• Salbutamol 100 micrograms CFC free inhaler
• Disposable spacers

We will always use your child’s own asthma inhaler and spacer as a first preference. The inhaler contained in the School’s Emergency Asthma Kit will only be used if your child’s asthma inhaler runs out or has expired when needed.

St George’s University Hospital has also provided Sellincourt with a Schools’ Emergency Allergy Kit including a

• Adrenaline 150 micrograms pre-filled pen
• Adrenaline 300 micrograms pre-filled pen for children 6 years and older

We will always use your child’s own Adrenaline pre-filled pen as a first preference. The pre- filled pen contained in the School’s Emergency Allergy Kit will only be used if your child’s Adrenaline pen is faulty or has expired when needed.
